Revised Form - Notice To Added Creditor(s) Effective Immediately

The Notice to Added Creditor(s) form has been revised and is available as a fillable form on the Bankruptcy Forms page of the Court’s website.  The form was revised to be applicable when creditors are added by filing an amended matrix and/or amended schedules, along with other non-substantive changes.

Instructions for filing an Amended Creditor Matrix and filing Amended Schedules D, E/F are also posted with the revised form.

Counsel for Debtor(s) should keep in mind the following when adding creditors to a bankruptcy case:

The Notice to Added Creditor(s) form is required if the 341(a) Meeting of Creditors Notice has been filed/docketed.

When adding creditors after Schedules D and E/F are filed, the filer must file the applicable amended schedule(s), an Amended Creditor Matrix and update the Creditor Database.  To avoid multiple filing fees associated with this scenario, see the instructions for Filing Amended Schedules D, E/F to Add Creditors.
