The Notice to Added Creditor(s) form has been revised and is available as a fillable form on the Bankruptcy Forms page of the Court’s website. The form was revised to be applicable when creditors are added by filing an amended matrix and/or amended schedules, along with other non-substantive changes.
Instructions for filing an Amended Creditor Matrix and filing Amended Schedules D, E/F are also posted with the revised form.
Counsel for Debtor(s) should keep in mind the following when adding creditors to a bankruptcy case:
The Notice to Added Creditor(s) form is required if the 341(a) Meeting of Creditors Notice has been filed/docketed.
When adding creditors after Schedules D and E/F are filed, the filer must file the applicable amended schedule(s), an Amended Creditor Matrix and update the Creditor Database.
