Effective September 1, 2020, the following documents (if applicable) are due with the voluntary petition:

• List of Creditors (“Matrix”);
• Verification of Matrix;
• Credit Counseling Certificate or Other Statement Pursuant to § 109(h);
• Statement of Social Security Number;
• Corporate Ownership Statement.

Any extension of time to file documents due with the voluntary petition may be granted only on motion for cause shown pursuant to the Federal Rules.

ECF filers will receive a notice to correct filing deficiencies for the documents listed above within TWO DAYS. Failure to timely cure deficiencies may result in dismissal of the bankruptcy case.
